Resumo

A hipertensão arterial é um dos principais fatores de risco cerebrocardiovascular modificáveis com expressão acentuada em populações rurais e envelhecidas. A hipotensão ortostática representa igualmente uma alteração hemodinâmica relevante nestes contextos. Integrado no Programa de Pressão Arterial na Beira Baixa, o presente estudo teve como objetivo principal determinar a prevalência de hipertensão arterial e de hipotensão ortostática nos concelhos de Belmonte e Penamacor, bem como analisar a sua associação com os principais fatores de risco cerebrocardiovasculares. A prevalência global de hipertensão arterial foi de 55,8%, sem diferenças significativas entre concelhos ou sexos. A idade constituiu o principal determinante sociodemográfico (p<0,001), observando-se um aumento progressivo da prevalência de hipertensão arterial com o envelhecimento: desde 19,6% no grupo etário dos 18-29 anos até 75,9% nos indivíduos de 70-79 anos. A obesidade, a diabetes mellitus e a hipercolesterolemia associaram-se significativamente à hipertensão arterial (p<0,001). Na análise de regressão logística, a idade, o índice de massa corporal, a diabetes mellitus e a hipercolesterolemia mantiveram-se como determinantes independentes da doença. Dos participantes medicados (36,5%), apenas 55,1% apresentaram controlo tensional adequado. Apesar da ausência de de diferenças na prevalência global de hipertensão arterial entre concelhos (p=0,331), os subtipos clínicos divergiram: Belmonte registou mais hipertensão arterial não controlada entre medicados (20,8% vs 13,6%; p=0,014) e Penamacor mais hipertensão arterial não diagnosticada (22,6% vs 14,2%; p=0,008), sendo o subdiagnóstico quase duas vezes mais frequente nos homens (p<0,001). A prevalência de hipotensão ortostática foi de 7,7%, associando-se à hipertensão arterial (p=0,009) e à diabetes mellitus (p=0,025). Apesar da ausência de diferenças intermunicipais na prevalência de hipotensão ortostática quando analisada como variável dicotómica, a magnitude da resposta hipotensora ortostática foi significativamente mais acentuada em Belmonte (p<0,001). Conclui-se que a hipertensão arterial apresenta elevada prevalência nestes territórios rurais, encontrando-se fortemente associada à idade e à carga cardiometabólica. Os resultados obtidos evidenciam a necessidade de reforçar as estratégias de rastreio, diagnóstico e controlo da doença ajustadas ao contexto local.
Abstract : Arterial hypertension is one of the main modifiable cerebrocardiovascular risk factors, with a marked impact in rural and ageing populations. Orthostatic hypotension also represents a relevant hemodynamic disorder in these settings. As part of the Beira Baixa Blood Pressure Program, the main objective of this study was to determine the prevalence of arterial hypertension and orthostatic hypotension in the municipalities of Belmonte and Penamacor, as well as to analyze their association with the main cerebrocardiovascular risk factors. The overall prevalence of arterial hypertension was 55,8%, with no significant differences between municipalities or sexes. Age was the main sociodemographic determinant (p < 0,001), with a progressive increase in the prevalence of arterial hypertension with ageing, ranging from 19,6% in the 18–29-year age group to 75,9% among individuals aged 70–79 years. Obesity, diabetes mellitus and hypercholesterolemia were significantly associated with arterial hypertension (p < 0,001). In the logistic regression analysis, age, body mass index, diabetes mellitus and hypercholesterolemia remained independent determinants of the disease. Among participants receiving antihypertensive medication (36.5%), only 55.1% had adequate blood pressure control. Although the overall prevalence of arterial hypertension did not differ between municipalities (p = 0,331), clinical subtypes differed: Belmonte showed a higher proportion of uncontrolled arterial hypertension among treated participants (20,8% vs 13,6%; p = 0,014), whereas Penamacor showed a higher proportion of undiagnosed arterial hypertension (22,6% vs 14,2%; p = 0,008), with underdiagnosis being almost twice as frequent in men (p < 0,001). The prevalence of orthostatic hypotension was 7,7% and was associated with arterial hypertension (p = 0,009) and diabetes mellitus (p = 0,025). The actual magnitude of the blood pressure drop was significantly greater in Belmonte (p < 0,001), although dichotomized orthostatic hypotension did not differ between municipalities. In conclusion, arterial hypertension showed a high prevalence in these rural territories and was strongly associated with age and cardiometabolic burden. These findings highlight the need to strengthen screening, diagnosis and disease control strategies adapted to the local context.
