Resumo

Introdução: As síndromes coronárias agudas são uma das principais causas de mortalidade cardiovascular, sendo o enfarte agudo do miocárdio uma das manifestações mais graves. A angioplastia de urgência, associada à troponina I de alta sensibilidade e alterações eletrocardiográficas, é essencial otimizar o prognóstico em pacientes com STEMI. Objetivo: Avaliar a relação entre a troponina I de alta sensibilidade e as alterações eletrocardiográficas em doentes com EAM com supradesnivelamento do segmento ST submetidos a angioplastia coronária urgente. Metodologia: Estudo observacional retrospetivo na Unidade de Intervenção Cardiovascular da ULS Cova da Beira, com indivíduos admitidos entre fevereiro de 2024 e janeiro de 2025, com dados das plataformas SClinic© e CardioBase©. A análise estatística incluiu testes não paramétricos e qui-quadrado, significância de 0,05. Resultados: Amostra maioritariamente masculina e de idade avançada, com elevada prevalência de hipertensão e dislipidemia. O EAM prévio associou-se a menor pico de troponina; os restantes fatores de risco, o território de enfarte e a extensão da doença coronária não se associaram ao biomarcador. A amplitude do supradesnivelamento em DIII e V2 correlacionou-se com o pico de troponina nos territórios inferior e anterior, respetivamente, com redução significativa em DIII e aVF após a intervenção. Conclusão: O EAM prévio foi o principal determinante do pico de troponina. A correlação entre a amplitude do supradesnivelamento no território culpado e o pico de troponina sugere que o ECG reflete a extensão da necrose miocárdica, sendo a sua redução após a ICP sugestiva de reperfusão eficaz. São necessários estudos prospetivos, com maior amostra e registo sistematizado.
Abstract : Introduction: Acute coronary syndromes are one of the leading causes of cardiovascular mortality, with acute myocardial infarction being one of the most severe manifestations. Emergency angioplasty, in conjunction with high-sensitivity troponin I and electrocardiographic changes, is essential for optimizing the prognosis in patients with STEMI. Objective: To evaluate the relationship between high-sensitivity troponin I and electrocardiographic changes in patients with STEMI who underwent urgent coronary angioplasty. Methodology: A retrospective observational study conducted at the Cardiovascular Intervention Unit of the Cova da Beira Regional Health Unit (ULS Cova da Beira), involving patients admitted between February 2024 and January 2025, using data from the SClinic© and CardioBase© platforms. Statistical analysis included nonparametric and chi-square tests, with a significance level of 0.05. Results: The sample consisted predominantly of older men, with a high prevalence of hypertension and dyslipidemia. Previous AMI was associated with a lower peak troponin level; the remaining risk factors, the infarct territory, and the extent of coronary artery disease were not associated with the biomarker. The amplitude of the ST-segment elevation in leads DIII and V2 correlated with peak troponin levels in the inferior and anterior territories, respectively, with a significant reduction in leads DIII and aVF following intervention. Conclusion: The pre-procedure ECG was the main determinant of the troponin peak. The correlation between the amplitude of the supranormal elevation in the affected territory and the troponin peak suggests that the ECG reflects the extent of myocardial necrosis, and its reduction after PCI is suggestive of effective reperfusion. Prospective studies with a larger sample size and systematic data collection are needed.
